Contact Form - Modified to make a Survey
With Sandvox 1.5 more field types and options have been added to the Contact Form page/pagelet. So, I've had a go at making a simple quiz/poll/feedback form using the new Sandvox Contact Form.
You will notice that the form below has no need for the end-user to enter a name or email address. All these fields have been hidden and filled with a default (but meaningful) 'Value'.
Note:

-->You must have your email address as the Send to address
You need to use your email address so that you can receive the sent emails.
-->You *must* have a valid Return email address 'format' as the Value
If you are going to hide the email field. The default Value is: email@domain.com . Anything in the 'dummy@dummy.com' format as the Value will work when the email field is hidden. Personally, I use one of my own email addresses that I use for testing only.
In the Sandvox created 'feedback form' below, I have hidden the Name, Email, Subject and Message fields of the form in the Sandvox Inspector and also:

-->The resulting email has the Sandvox 'thing' of using the whole label in the submitted reply, with_underscores_replacing_any_spaces, for added illegibility.
-->To add 'Choices' in the Radio or Popup Button Type of field: Type the Choice and tap the Return key, tap the Return key a second time to refresh the field choices on the form page in Sandvox.
Tip:
For longer questions and choices it may be easier to use copy and paste from a text file rather than type the text directly into the Label or Choices field. Labels can be edited but once a Choice becomes a button I needed to delete it and re-type it to 'edit' the Choice text.
